The Live Active Referral Scheme is a joint initiative between NHS Greater Glasgow and West Dunbartonshire Council. It also operates in Glasgow City Council, East Dunbartonshire Council, South Lanarkshire Council, East Renfrewshire Council and Glasgow Caledonian University. It was established initially as the GP Referral Scheme within Glasgow City Council in 1997 and then extended to West Dunbartonshire in 2001. In 2008 the scheme expanded to incorporate all west Dunbartonshire areas by using financial support from the Keep Well project.
Keep Well is a national project with the aim of improving the health of those who need it most. In West Dunbartonshire we will be offering health checks to individuals with Coronary Heart Disease from participating GP practices. The health check appointment lasts around 45 minutes and based on this health check the patient will be offered a range of services which aim to reduce the risk of deterioration in their condition and help maintain quality of life.
