Third Party Claim Form

This Incident Report Form should only be completed where it can be demonstrated that, any loss of or damage to personal property or personal injury, strictly and specifically has occurred.

If you are insured for this type of loss then you should consider intimating the claim to your own insurers in the first instance. Where appropriate, they may then claim against the Council.

It should be noted that all relevant sections on the Incident Report Form must be completed and any requisite documentation i.e. purchase receipt, quotation, medical evidence etc. submitted otherwise the processing of your claim could be delayed.

Property Claim Form

This Property Claim Form should only be completed if you are a tenant of a Council owned commercial/industrial property which has sustained damage and, in terms of your lease, the Council arranges the building insurance.

It should be noted that all relevant sections on the Property Claim Form must be completed and any requisite documentation i.e. repair estimate etc. submitted otherwise the processing of the claim could be delayed.

A £250 policy excess is applicable in respect of each and every loss. Responsibility for payment of any amount up to £250 rests with the tenant.
