Community Health and Care Partnership

The Community Health and Care Partnership (CHCP) is a Partnership between West Dunbartonshire Council and NHS Greater Glasgow and Clyde and brings together both NHS and Local Authority responsibilities for community-based health and social care services within a single, integrated structure.

 

 

The CHCP came into being on the 1st October 2010, and will build on the strong local track record of joint working between the staff and services of West Dunbartonshire Council and NHS Greater Glasgow and Clyde.

GOOD MORNING CLYDEBANK

A valuable service that operates in the West Dunbartonshire area, facilitated by Clydebank Seniors’ Forum:

 

“Good Morning Clydebank”

 

This invaluable service has been operating successfully in the West Dunbartonshire area for some time, and is available to anyone who is elderly, frail or housebound and who would welcome a friendly chat between 9.30 am and 11 am Monday to Friday.

 

A telephone call is made to the individual to ascertain how they are and to have a chat, and to see if any help or assistance is required.  Information on resources and services provided by West Dunbartonshire Council can also be given.

 

Past and present users of the service find this free service of daily contact comforting and helpful, especially given that for many, this might be the only contact with the outside world they have, apart from their home helps.

 

If you have any referrals who might find the service beneficial, please contact me on the “Good Morning” telephone line 01389 380387.

 

I would require the following information for any referral made; the name, address and telephone number of the recipient along with their date of birth and details of their next of kin.
